Four Easy Steps to Complete Your Charitable Gift Annuity

Request an Illustration by completing an Illustration Request Form provided to you by the Foundation. Your illustration will be returned to you quickly and will include information such as annuity rate, estimated charitable tax deduction, and the likely taxation schedule of your annuity payments.

Review the Illustration and the disclosure statement provided to you with your personal financial, tax, and legal professionals.

Complete Your Gift by contacting the Foundation at 734-663-0401. The Foundation can provide details for you to make your gift and will begin to prepare final documents.  

    Your contract will be established on the date your check or wire transfer is received by the Foundation. Once the date of your gift is known, all contract paperwork can be established in advance of making your gift.
    The Foundation will provide you or your financial advisor appropriate Transfer and Delivery Instructions to make your gift. The precise value of your gift will vary based upon the median value of the high and low trading price on the date of transfer. Final paperwork can only be produced subsequent to valuing your gift – information usually available the next business day. 

Sign and complete the documents. You will need to provide the following information in order to complete your charitable gift annuity contract and ensure accurate delivery of annuity payments and an annual 1099R tax statement.

  • ­COST BASIS INFORMATION for gifts of marketable securities (if applicable)
  • ­ACH DEPOSIT INFORMATION (optional) if you wish your annuity payments to be directly deposited into your preferred account
